The Head of Service has acknowledged procedural failures linked to the PFIPC scandal after lawmakers uncovered a series of questionable approvals and forged documents.During a session with the House of Representatives, the HoS admitted that due diligence was not properly carried out on several files tied to the alleged scheme.
The admission came as members of the House committee presented evidence suggesting that approvals for the purported agency were obtained outside of standard government processes. The lawmakers said their investigation revealed a network of irregularities. According to the committee, some of the documents submitted in support of the PFIPC were fake, while others were signed off without following due process. The Reps described the situation as a breach of public service rules that allowed unauthorized dealings to scale through.
The Head of Service told the committee that the lapses had created room for abuse and promised that corrective measures would be taken. He noted that the Office of the Head of Service would review internal controls and strengthen verification procedures to prevent a recurrence. He also pledged cooperation with the ongoing legislative probe.
The House Committee said it would continue its investigation and forward findings to relevant anti-graft agencies. Lawmakers stressed that those found culpable, whether in the civil service or outside, must be held accountable. They added that restoring public confidence would require sanctions and reforms in how approvals are granted.
